Öğe Clinical and radiographic assessment of the association between orthodontic mini-screws and periodontal health(2024) Yıldız, Mehmet Selim; Acar Ulutaş, Pelin; Özenci, İlknur; Akçalı, AliyeObjectives: Proper anchorage control is crucial for predictable tooth movement and preventing inadequate torque during orthodontic treatment. Through clinical and radiographic parameters; this study assesses the association between mini-screws and periodontal health. Materials and methods: A prospective observational study included 16 systemically healthy non-smoking individuals requiring mini-screws. Mini-screws with a rough, titanium oxide-coated surface were placed. Periodontal assessments (Plaque index, gingival index, probing pocket depth, gingival recession, bleeding on probing, mucosal discomfort, mucosal redness, keratinized tissue width, supracrestal tissue height, and transmucosal soft tissue thickness) were performed at 2nd week and 3 months post-placement. Radiographic evaluations measured distances between mini-screws and adjacent teeth using Image J software. Results: The study included 13 females and 3 males (mean age 21.9 ± 1.8 years) with 24 mini-screws. Early mini-screw loss was not observed. Significant reductions in site-level Gingival Index and bleeding on probing (p < 0.05) and full-mouth bleeding on probing (p < 0.05) were noted over time. Absence of significant differences was found in mucosal discomfort and redness, keratinized tissue width, or transmucosal soft tissue thickness, but supracrestal tissue height decreased significantly (p < 0.05). Radiographically, significant bone reduction around mini-screws was observed at 3 months, with torque gauge values significantly decreased as well (p < 0.05). Conclusions: Orthodontic mini-screws can be effectively utilized in orthodontic treatment with proper planning and monitoring. While improvements in gingival health were observed with targeted oral care, the study underscores the need for careful consideration of potential risks to periodontal tissues, such as reductions in supracrestal tissue height and bone levels. A balanced approach that integrates preventive strategies with precise screw placement is essential to maximize the benefits of mini-screws while minimizing potential periodontal complications. Clinical relevance: While proper oral hygiene can help control inflammation around mini-screw sites, clinicians must also be mindful of potential risks, such as reductions in bone levels and tissue height. Careful patient selection, precise placement, and regular follow-up are crucial to ensure the stability of mini-screws' stability and to prevent complications, ultimately contributing to better treatment outcomes in orthodontic care. It is critical that today's healthcare students have the right knowledge and perspectives on HIV/AIDS, as they are the first line of defense against such a threat locally and globally. This cross-sectional study was carried out to evaluate the knowledge and attitudes of healthcare students toward HIV/AIDS. Method: In this study, a 4-part questionnaire consisting of 50 questions was administered to 450 healthcare students. Participants’ sociodemographic status, general knowledge of HIV/AIDS, their attitude to patients, and their knowledge related to oral manifestations of it were evaluated. Results: With the participation of 100 students from each of the faculties of dentistry, medicine, and pharmacy, a response rate of 66.7% was achieved. The mean knowledge of HIV/AIDS score percentage was 44.2% in dentistry, 43.3% in medicine, and 44.6% in pharmacy. It was determined that they had a positive attitude towards HIV/AIDS patients, and their mean attitude percentage was 78.6% in dentistry, 75.9% in medicine, and 76.2% in pharmacy. When it comes to the oral manifestations of HIV/AIDS, as expected, dentistry students were found to have higher scores on the most common oral manifestations. Still, it was observed that students of all three faculties were not aware of most lesions. Conclusion: Although students' knowledge levels were lower than expected, it was determined that most students displayed a professional attitude towards HIV/AIDS. The results obtained from this study revealed that dentistry, medicine, and pharmacy students need more detailed relevant education.Öğe Knowledge and Attitudes of Oral Health Professionals to Different Dental Treatment Approaches in Bisphosphonate Therapy(Ataturk Universitesi, 2025) Dedeoğlu, Yasemin; Özenci, İlknur; Ahmedbeyli, Cavid; Çakar, Gökser; Dirikan İpci, ŞebnemObjective: This study evaluates oral healthcare professionals' attitudes towards managing different dental clinical scenarios considering different durations and administration routes of antiresorptive drugs. Methods: This study is a cross-sectional, Web-based survey. The first part of the survey evaluated demographic data; the second part investigated dentists' medication-related osteonecrosis of the jaw (MRONJ) treatment approach; the third part comprised clinical treatment scenario questions regarding the administration route and duration of usage. Oneway ANOVA test was used in the intergroup comparison of parameters showing normal distribution, and Tamhane's T2 test was used to determine the group that caused the difference. Student t-test was used for comparison of parameters showing normal distribution. The chi-square test, Continuity (yates) correction, and Fisher Freeman Halton test were used to evaluate qualitative data. Significance was set at P <.05. Results: 35.8% of dentists said they would not treat at-risk patients as the correct approach. More than half of the participants prefer to refer to the cases of osteonecrosis from the 1st stage to a specialist. A statistically significant difference was found between the correct answers about all five treatment scenarios for patients receiving oral bisphosphonate (BP) ≤3 years in favor of experienced dentists (P<.05). Conclusion: Within the confines of this study, dentists' knowledge of BP and osteonecrosis is moderate. Dentists with less than ten years of working experience are more cautious about patients using BP due to the risk of developing osteonecrosis when different dental treatments are performed.Öğe Treatment of iatrogenic factor related gingival recession: A case report(Altınbaş Üniversitesi, 2020) Özenci, İlknur; İpçi, Şebnem Dirikan; Çakar, Gökser; Yılmaz, SelçukRecent reports in the literature show that the application of coronally advanced flap i n c ombination with subepithelial connective tissue graft in the treatment of Miller Class II and even III gingival recessions leads to successful results. This case report describes the use of subepithelial connective tissue graft and enamel matrix derivative combination with coronally advanced flap for the treatment of localized gingival recession caused by an ill-fitting crown restoration. The restorative treatment comprised of the retreatment of the endodontic and restorative needs. At 12 months, successful root coverage with 3.5 mm attachment gain was achieved. The patient had minimal discomfort and was satisfied with esthetic results. This case encourages the usage of subepithelial connective tissue graft, enamel matrix derivative and coronally advanced flap combination in localized Miller Class III gingival recessions. Even though the treatment of Miller Class III gingival recessions are challenging due to loss of interproximal bone and soft tissues, the application of subepithelial connective tissue graft in combination with the use ofenamel matrix derivative has shown to be a predictable treatment procedure in esthetic areas forroot coverage and gain of clinical attachment in single Miller Class III recession defects.Öğe Treatment of multiple gingival recessions by tunnel technique and acellular dermal matrix graft: a case report(Altınbaş Üniversitesi, 2018) Özenci, İlknur; İpçi, Şebnem Dirikan; Yılmaz, Selçuk; Çakar, GökserSuccessful clinical outcomes have been achieved with various root coverage procedures for the treatment of Miller Class I and II multiple gingival recessions. Regarding Miller Class III recessions, unpredictable success rate was reported due to the loss of interproximal tissue support. Recently, it has been demonstrated that complete root coverage is possible in Miller Class III defects, when the defects present mild interdental attachment loss. This case report describes the application of acellular dermal matrix graft (ADM) and tunnel technique (TUN) combination, for the treatment of Miller Class III multiple gingival recession defects. At 12-month, complete root coverage on teeth #41 and partial root coverage on teeth #31 with mean root coverage of 83.3% were achieved. Root coverage esthetic score was 7 with successful esthetic results. Besides significant defect coverage increase in keratinized tissue height and gingival thickness were obtained. According to the present clinical case, Miller Class III multiple gingival recessions can be predictably treated with TUN+ADM, if the attachment loss is not too severe.
